Diabetes Care
The goal of our team is to provide a level of care for the diabetic patient that is goal driven and provides the patient the ability to care for him/herself.
Education of the patient and caregiver is most important. Nightingale provides oral instruction as well as various written materials that help patients understand their disease, the care they must provide for themselves, and complete understanding of what must be done to maintain and keep the disease process at bay.
Dietary teaching is essential. This includes:
- Meal planning
- What not to eat
Our team accomplishes diabetic care through:
- Educational programs
- Insulin administration
- Self-care awareness
- Diabetes the disease process
- How to live with diabetes
- What can I do?
- Monitoring blood glucose levels (at home testing)
- Laboratory monitoring
- Appropriate health examinations
- Dietary teaching
- Utilization of state-of-the-art equipment
- Recognition of potential complications and providing a plan of action
- Diabetic Foot Care: including monitoring for skin lesions on the lower extremities and patient/caregiver education on proper foot care
The Nightingale Home Healthcare Diabetes Care program provides a strong strategy to help people with diabetes. Our licensed nurses, physical therapists, and occupational therapists provide a comprehensive interdisciplinary approach that ensures the most successful outcome for our patients’ needs.
