  • Our pharmacist performs medication reviews for Nightingale patients in Indiana, Illinois, Minnesota, California, and Nevada, and also Texas Aspire, and Nightingale Visiting Nurses to identify potential drug irregularities. These irregularities are communicated to clinicians and physicians when appropriate. Findings may include:
    1. Duplicate therapy
    2. Potential side effects which may interfere with care
    3. Potential drug interactions
    4. Requesting lab orders to help manage medication therapy
    5. Identifying patients who might benefit from HomMed telemonitor or Med Partner Dispenser
  • Our pharmacist also communicates directly with the Telemedicine nurse to manage medication related problems.
  • Tracks average medication use for Nightingale Home Healthcare patients.
  • Manages and organizes annual flu vaccine campaign by:
    1. Developing policies and procedures for handling, documentation, storage, and supply of vaccine.
    2. Providing in-service for appropriate staff regarding vaccine administration, documentation, and reaction preparedness
  • Our pharmacist is also a Certified Immunizing Pharmacist and therefore administers vaccines.
  • Educates staff with timely, useful information regarding medications.
  • Maintains clinician resource library that includes information about diabetes management, food-drug interactions listings, dietary supplements, wound treatment, and medications that interfere with wound healing.
  • Provides "brown bag" medication information sessions at senior living facilities about medications—uses, generics, OTC, food/drug interactions.
  • Educates staff with timely, useful information regarding medications.
  • Contributes educational studies to Nightingale University.
