Home Health Office / Triage LPN - Indianapolis, IN

We are currently seeking to fill up a position for full time LPN Office / Triage role. In this position, you will assist with the coordination of care through the coordination of various patient, physician, and family activities and communications. Your role will also include regulatory compliance as well as fiscal management responsibilities.

Essential Duties include:

• Receiving telephone calls and triaging situation from, skilled field staff
• Medication alerts reporting and follow-up with staff
• Call logging for missed visits and identification of MD notifications
• Update orders
• Enter of physician orders when they come through
• Complete census reports
• Documentation issues / notes
• Tracking / coordinating visits

Requirements include:

• LPN license required
• One year minimum of home health nursing experience is preferred but not required
• Two year minimum of LPN patient care experience is required
• One year minimum of office / management task utilizing the above tasks is required
• Able to use electronic medical records
• Comfortable using a computer / pulling reports
• Open to make patient visits in the field if needed
• Able to participate in the field on call rotation with needed
